Replacement of a Degree Certificate Form [PDF] You must complete the equivalent of a Statutory Declaration in the country you are residing. Your Statutory Declaration must include all of the following: - Full Name
- Date of Birth
- Address
- Telephone Number
- Degree Title (e.g. Bachelor of Commerce)
- Conferral Date (date of graduation e.g. 21 April 1984)
- State if your degree has been lost, stolen or destroyed
- Agree that if the original degree is found you will return one copy to the University
- Have your signature witnessed by a notary or equivalent
