For requests made by graduates who are not Australian Citizens and living overseas please complete the following form.

Replacement of a Degree Certificate Form [PDF]

You must complete the equivalent of a Statutory Declaration in the country you are residing.

Your Statutory Declaration must include all of the following:

• Full Name
• Date of Birth
• Address
• Telephone Number
• Degree Title (e.g. Bachelor of Commerce)
• Conferral Date (date of graduation e.g. 21 April 1984)
• State if your degree has been lost, stolen or destroyed
• Agree that if the original degree is found you will return one copy to the University
• Have your signature witnessed by a notary or equivalent

Australian Citizens Residing Overseas

For requests made by Australian Citizens residing overseas please use the following form.  This Statutory Declaration must be completed at an Australian Embassy or Consulate.  If you are unable to access the embassy please use the request for graduates residing overseas.
